Our professional referral process is designed to be clear, efficient and supportive. From submitting the referral through to assessment and placement, our team will review the individual’s housing circumstances, support needs, risks and suitability for available supported accommodation.

Once the referral is received, our team will carefully assess the information provided. We may contact you to clarify details, request supporting documents or discuss the individual’s needs so we can determine whether our accommodation and support model may be suitable.

Step 3: Suitable Housing Placement 

If the referral is accepted and suitable accommodation is available, we will work with you and the individual to arrange an appropriate placement. We will consider their support needs, safety, location and the type of accommodation available before confirming the next steps.

We accept referrals from professionals and organisations supporting adults who may need safe, stable supported accommodation. Referrers should have a good understanding of the individual’s current housing situation, support needs, risks and personal circumstances, so our team can carry out a fair and informed assessment. Providing clear and accurate information helps us understand whether our accommodation and support model may be suitable and allows us to respond as efficiently as possible.​

We accept referrals for adults who may need safe, stable supported accommodation and practical help to maintain their housing. The person being referred should have low to medium support needs and be willing to engage with the support available. Our team will consider their current housing situation, level of independence, identified risks and whether our accommodation can meet their needs safely and appropriately.

Our Professional Referral Assessment Process

Referral Screening

When the referral form reaches our team, we begin by checking the information supplied. We consider the person’s housing circumstances, current support requirements, known risks and any urgent concerns to establish whether their needs could be met within our supported accommodation service.

Placement Suitability

Our team then considers whether the proposed placement would be safe, appropriate and beneficial for the individual. We review factors such as their independence, support needs, risks, preferred area and compatibility with the accommodation and level of housing related support available.

We understand that timely housing decisions are important for both the person being referred and the professionals supporting them. Our assessment process is designed to be clear, thorough and responsive. Once we receive a completed professional referral, our team will review the information carefully and aim to provide housing within 48 hours.

Discussion With Referrer

A member of our team may contact you to discuss the referral in more detail. This short conversation allows us to clarify important points, gain a fuller understanding of the person’s circumstances and answer any questions you may have about the proposed housing pathway.

Outcome Confirmation

Once the assessment is complete, we will update the referrer with the outcome. Where the referral is approved, we will outline the available placement and what needs to happen next. Where we cannot proceed, we will explain the decision as clearly as possible.

Supporting Information

To complete our checks, we may request relevant documents or further evidence from the referring organisation. This may include identification, risk information, support plans, professional reports or housing records. We will tell you exactly what is required and how it supports the assessment.

Arranging the Move

When suitable accommodation is available, we coordinate the move with the referrer and the person being placed. Our team will confirm the practical arrangements, share important information and provide a welcoming introduction to the property and the support available after arrival.
