Vulnerable Adults in Housing Crisis

We support adults whose housing, support network or personal circumstances have broken down unexpectedly. A stable home and practical support can help them rebuild confidence, routine and greater independence.

Low to Medium Support Needs

Our service may suit adults who can live with some independence but need help with routines, budgeting, appointments, wellbeing or maintaining their accommodation. Support is shaped around individual needs and goals.

Needing Greater Stability

We may help adults who have no settled home, are moving between short term arrangements or are struggling to maintain their current accommodation. Our supported housing offers structure, consistency and practical support.

Leaving Unsafe Accommodation

We aassist adults who cannot safely remain in their current home because of abuse, exploitation, intimidation, violence or another serious concern. Each referral is reviewed carefully to understand the risks involved.

Immediate Risk of Homelessness

We help people who could lose their home because of eviction, family breakdown, financial pressure or another sudden change. Early access to supported accommodation can provide security before the situation worsens.

 Experiencing Homelessness

We support adults who are homeless, sleeping rough, sofa surfing or living without secure accommodation. Our aim is to provide a safe home, practical guidance and the stability needed to begin moving forward.

Common Eligibility Criteria

Every referral is considered individually, but there are some common factors we look at when deciding whether our supported accommodation may be suitable. Our aim is to make sure each person can live safely within the property and receive the right level of practical support.

We consider the person’s age, housing need, independence, support requirements, known risks and willingness to engage. Meeting the criteria does not automatically guarantee accommodation, as every placement is also subject to assessment and availability.

Eligibility Criteria Includes:

  • Is aged 18 or over

  • Needs supported accommodation

  • Has low to medium needs

  • Can live semi independently

  • Is willing to engage

  • Can follow house rules

  • Can share housing safely

  • Has manageable identified risks

  • Meets relevant funding criteria

  • Is suitable for available housing

Every referral is reviewed carefully to make sure our supported accommodation and level of support are appropriate for the individual. We consider their housing needs, independence, known risks and the type of placement available before making a decision.

Step 1: Referral Reviews

We begin by reviewing the information provided about the person’s housing situation, support needs, current circumstances and any known risks. This helps us understand whether our service may be suitable and whether more information is needed.

How We Assess Suitability

Step 2: Further Assessment

Where required, we may speak with the individual, referrer or professionals already involved in their support. This allows us to clarify important details, understand their needs more fully and consider how safely they could be supported.

Step 3: Placement Decision

Once the assessment is complete, we decide whether the accommodation and support available are appropriate for the person. If a suitable placement is available, we will explain the next steps clearly and begin arranging the move.

When Our Service May Not Be Suitable

We want every person to be placed somewhere they can feel safe, supported and properly understood. In some situations, our supported accommodation may not be able to provide the level of care, supervision or specialist help a person needs to live safely and make positive progress.

This does not mean the person is not deserving of support. It may simply mean that a different setting, care provider or specialist service would be better suited to their circumstances. Where possible, we will explain our decision clearly and help identify another route that may be more appropriate.

Eligibility Criteria Includes:

  • Needs continuous personal care

  • Requires intensive supervision

  • Needs specialist medical support

  • Requires secure accommodation

  • Has risks that cannot be managed

  • Needs support beyond our level

  • Cannot live semi independently

  • Cannot share accommodation

  • Requires clinical oversight

  • Is unable to engage with support
